                         Caregiving from a Strengths-Based Perspective
 
My name is Rajiv Chaku. I am a fourth-year student in the Spalding School of Professional Psychology, at Spalding University in Louisville, KY. You are invited to participate in my research entitled: A Proposed Screener: The Caregiver Relationship Inventory, Caregiving from a Strengths-Based Perspective. This consent form will explain the details of participating in this research. Please read this form carefully. Feel free to contact us if you have any questions about the study.
 
What are we trying to find out in this study?
The purpose of this study is to explore how the relationship between a caregiver and the person being cared for relates to caregiver burnout. You will respond to a series of questions within a survey asking about your relationship as well as demographic questions.
 
Who can participate in this study?
You must be at least 18 years old and 1) give ongoing help to someone who needs care 2) you do not get paid or get paid very little for helping take care of this person 3) you help this person with daily tasks like bathing, getting dressed, making meals, eating, cleaning, and other things they cannot do on their own, and 4) you provide this person help at home which does not include places like hospitals, nursing homes, etc.
 
Where will this study take place?
This study is an online survey.
 
What is the time commitment for participating in this study?
The survey should take about 20 minutes to complete.
 
What will you be asked to do if you choose to participate in this study?
You will be asked to respond to 47 questions.
 
What are the risks of participating in this study and how will these risks be minimized?
There are minimal risks to you in this study. Some questions may bring up strong feelings for you due to the sensitive nature of caregiving. However, questions have been carefully phrased to accommodate this potential for sensitivity. Mental health resources will also be provided to you to minimize any emotional distress. Another risk may be if your data was to be disclosed. However, responses to this survey will be anonymous and unable to be traced back to you. You will have the option to discontinue this survey at any point without consequence.
 
What are the benefits of participating in this study?
There are no direct benefits to you. However, your participation will help us to better understand how the relationship between a caregiver and a person they care for is related to burnout in caregivers. This insight can help to further our understanding about caregiver burnout.
 
Are there any costs associated with participating in this study?
There are no costs to participate in this study.
 
Is there any compensation for participating in this study?
Participants who complete the entire survey will be able to participate in a raffle for a $50 gift card.
 
How will my data be protected?
All information obtained is completely voluntary. Only the investigators have access to your responses and informed consent. This data will only be used to support the goals of this current project. Only summary data will be reported in any conference and/or paper. Your name will not be collected / be associated with your responses. All electronic information will be stored for a minimum of three years after the study is completed. Data will be stored in a password protected file on a password protected computer in a locked office on Spalding University’s campus.
 
What if you want to stop participating in this study?
Your participation in this study is completely voluntary. You can leave the study without penalty. Please contact Rajiv Chaku (rchaku@spalding.edu) or Dr. Lee Penn (lpenn01@spalding.edu) if you have any questions about the study. If you have any questions or concerns about your rights as a research volunteer, you can contact Dr. Lisa Potts, Chair of the Research Ethics Committee at rec@spalding.edu. You can withdraw from the study at any time if you no longer wish to participate in this research. Signing below indicates your consent to have your data used for the purpose of this research project.
 
Please indicate you meet the following criteria to participate in this study:
I confirm I am at least 18 years old who gives ongoing help to someone who needs care.
I confirm I do not get paid or get paid very little for helping take care of this person.
I confirm I help this person with daily tasks like bathing, getting dressed, making meals, eating, cleaning, and other things they cannot do on their own.
I confirm that I provide this person help at home which does not include places like hospitals, nursing homes, etc.
